Important Notice: This Privacy Policy describes how Anchor Psychiatry, PLLC collects and uses information on this website. For information about how we handle your protected health information (PHI) as a healthcare provider, please refer to our HIPAA Notice of Privacy Practices provided to you at intake, or contact us directly at NicoleGuzman@anchor-psychiatry.com.

1. Who We Are

Anchor Psychiatry, PLLC ("Anchor Psychiatry," "we," "us," or "our") is a telehealth psychiatric practice based in Texas, operated by Nicole Guzman, PMHNP-BC, APRN, MSN. We provide 100% virtual psychiatric care to patients ages 10 and older across the state of Texas.

This Privacy Policy applies to information collected through our website at www.anchor-psychiatry.com (the "Site"). It does not govern clinical records or protected health information (PHI), which are handled in accordance with HIPAA and our Notice of Privacy Practices.

2. Information We Collect

Information You Provide Directly

When you contact us, submit a form, or initiate a booking through our Site, we may collect:

  • Your name and contact information (email address, phone number)
  • General information about why you are seeking care
  • Any messages or inquiries you send to us

Information Collected Automatically

When you visit our Site, we may automatically collect certain technical information, including:

  • IP address and general geographic location (city/state level)
  • Browser type and device information
  • Pages visited, time spent on pages, and referring URLs
  • Cookies and similar tracking technologies (see Section 5)

Information from Third-Party Booking

Appointment scheduling is handled through SimplePractice, a HIPAA-compliant patient portal. Information submitted through that portal is governed by SimplePractice's own privacy policy and our HIPAA Notice of Privacy Practices — not this document.

3. How We Use Your Information

We use information collected through this Site to:

  • Respond to your inquiries and appointment requests
  • Communicate with you about our services
  • Improve and maintain the Site's performance and content
  • Comply with applicable laws and regulations
  • Understand how visitors find and use our Site (analytics)
  • Run advertising campaigns on platforms like Google and Meta (Facebook/Instagram), where permitted

We do not sell your personal information to third parties. We do not use your information for purposes inconsistent with those described above without your consent.

4. How We Share Your Information

We may share your information only in the following limited circumstances:

  • Service providers: We work with trusted vendors (such as website hosting, analytics, and email services) who process data on our behalf under confidentiality obligations.
  • Legal compliance: We may disclose information if required by law, court order, or governmental authority.
  • Business transfers: In the event of a merger or acquisition, your information may transfer to the successor entity, subject to the same protections.

We do not share your information with advertisers in a way that identifies you personally.

5. Cookies & Tracking

Our Site uses cookies — small text files stored on your device — to remember your preferences (such as dark mode) and understand how visitors use the Site. We may also use analytics tools such as Google Analytics to collect aggregated, anonymized usage data.

You can control cookies through your browser settings. Disabling cookies may affect some Site functionality (such as theme preferences), but will not prevent you from accessing any content.

If we run Google or Meta advertising campaigns, those platforms may place cookies to measure ad performance. These cookies are governed by Google's and Meta's respective privacy policies.

6. HIPAA & Protected Health Information

Anchor Psychiatry is a HIPAA-covered entity. Any protected health information (PHI) you share with us in the context of receiving care — including information submitted through our SimplePractice patient portal — is handled in strict accordance with the Health Insurance Portability and Accountability Act (HIPAA) and our Notice of Privacy Practices.

If you have questions about how your clinical information is used or shared, please contact us at NicoleGuzman@anchor-psychiatry.com or text us at 972-630-0222.

7. Data Security

We take reasonable technical and organizational measures to protect the information we collect. Our Site uses TLS encryption (HTTPS) for all connections. Booking and patient data are handled through SimplePractice, which maintains its own HIPAA-compliant security infrastructure.

No method of transmission over the internet is 100% secure. We encourage you to contact us by phone or through our secure patient portal for any sensitive health-related communications.

8. Your Rights (Texas & Federal)

Depending on applicable law, you may have the right to:

  • Request access to the personal information we hold about you
  • Request correction of inaccurate information
  • Request deletion of your information, where permitted by law
  • Opt out of marketing communications at any time
